What we need to know at your first visit

You will be asked to complete a Patient registration Form prior to your appointment. This includes telling us about your medical conditions, medications, operations and if are under a Consultant for any reason.

These questions help us form a picture of your health and wellbeing, which will help us decide what treatments are appropriate for you. If you are not accurate in the answers that you provide, we will be unable to give you accurate advice or carry out our treatments safely, increased by factors such as poor quality of skin or if you are taking blood thinners (anticoagulants).

Should there be any changes to your health, medications or personal details, please advise us at your next appointment. We will always endeavour to ask about any changes when you attend the clinic.

Please bring a copy of your current medication prescription with you to clinic just in case.

 

Consent for Treatment

We are unable to treat your foot problem without your consent. Minors (under 16 years old) will require a parent or guardian to sign for their consent and accompany them to all treatments.

You will be asked to sign the Patient Registration Form to give your consent. By signing, you will be consenting to:

  1. The use of sharp instruments in your treatment. As such, there is a small risk of drawing blood. This risk can be increased by factors such as poor quality of skin or if you are taking blood thinners (anticoagulants). Some procedures may also carry a very slight risk of infection after treatment.  Certain advanced procedures may have other risks or side effects which will be explained in full prior to being carried out.  These treatments will normally have additional consent forms.
  2. Video analysis or photographic images taken of your lower limb may be necessary in order to track progress of some treatments.

Should you wish to withdraw your consent to treatment, please let the treating clinician know prior to treatment.

By signing the Patient Registration Form and any other consent forms, you are stating that you have your own right of consent for medical treatments.

 

Data Consent and Confidentiality

We will collect data from you to meet mandatory requirements regarding Medical Notes.

There is a legal obligation to keep medical records for a period of time after treatment. This can vary in length depending on your age and ability to consent, but will be so that we can provide you with an efficient service. in the administration of our business be for a minimum of 7 years following your last treatment.  After this period, your details will be destroyed.

As with all health and Medical Professions, we are bound by patient confidentiality. We do not share your personal information with a 3rd party. We will always ask your permission if we need to write to your doctor or other health professional regarding your foot health.

Only clinicians and reception staff working at Reid Podiatry have access to your patient record and personal details, which are kept electronically on a safe server.  Your paper Patient Registration Form is filed in a lockable metal cabinet so we have access to it in case of the internet failing.

We also collect data to assist us in the administration of our business to provide you with an efficient, effective and safe service.  From time to time we ask for patient feedback which may be given anonymously if you wish.

We may use your contact details to send you information about offers, new treatments or a newsletter. You may opt in to receiving this information and opt out at any time.

You can request a copy of any data held in your name by writing to the Clinic Owner at the clinic address.

 

Complaints & Accolades

In the unlikely event that you experience a problem after your treatment, please contact the clinic as soon as possible so that can assess and rectify the problem.  There is never a charge for a niggle check within a week of treatment.  If you contact us over a week with a problem, a charge may be made and this is at the discretion of the treating clinician.

Please refrain from leaving disparaging remarks on Social media without first giving us the opportunity to rectify the problem.

Should you wish to make a formal complaint about any aspect of the service at Reid Podiatry, please contact the Clinic Owner at the clinic by telephone or email.
